Breast augmentation, also known as augmentation mammoplasty, is a surgical procedure performed to enhance the shape and size of a woman’s breasts, using breast implants. This procedure can help to make the breasts more proportionate to the rest of the body, even those which are different in size or shape. Breast Augmentation can also enlarge and reshape breasts which have lost their fullness or shape due to weight loss, pregnancy, breast feeding, or age. Breast augmentation can also be used for reconstructive purposes on patients who have undergone mastectomy or those with a congenital breast deformity. The best candidate for breast augmentation is a woman 18 years or older who is unhappy with her breast size or shape and looking to alter their appearance. Dr. Messa has found that breast augmentation can not only enhance the look of the breasts, but has often had the added benefit of increasing one’s self confidence.

THE SURGERY

Breast augmentation typically takes approximately one hour to complete and may be performed under general anesthesia or a combination of local anesthesia and intravenous sedation. Dr. Messa offers several different possible locations for the incisions: axillary, which is under the armpit, inframmary, which is within the crease below the breast; and periareolar, which is around the nipple. Once he makes the incision, the breast tissue is lifted to make a space or pocket into which the implant is then inserted. The implant may be placed in one of two locations: subglandular, which is above the chest or pectoralis muscle, or submuscular, which is beneath the pectoralis muscle.

Breast Incision DiagramThere are two types of implants available for augmentation: saline filled or silicone gel. The saline implant is a silicone shell that is filled with a normal saline (intravenous fluid) solution. Most implants are inserted empty and then filled through a closed system utilizing a no touch technique, allowing for minimal scarring and decreased chance for infection. The silicone implant is a new, cohesive silicone gel implant that comes already filled. Dr. Messa uses the latest technology in implant design from Allergan, the Natrelle implant, and Mentor, the Memory gel implant, to ensure patient safety and satisfaction. After the implant is inserted, the incisions are then closed meticulously with several layers of absorbable sutures to decrease scarring . Steri-strips or adhesive tissue glue (Dermabond) are used for added support and the breasts are typically wrapped with gauze and an ace bandage during the initial 24 hours after the procedure.

Breast augmentation involves many different options and techniques. Choices must be made about the incision location, type of implant, and the implant placement. The choices that are right for you will depend upon your individual circumstances, and, thus, are best made with the help of Dr. Messa.. In general, submuscular placement, or beneath the chest muscle, typically involves a decreased risk of capsular contracture, implant palpability, and mammographic interference. Subglandular placement, the implants are placed above the chest muscle, may be utilized to lift as well as enlarge a sagging breast. However, it may also involve an increased risk of implant palpability, capsular contracture, and mammography interference depending on the individual patient’s breast and chest wall characteristics.

THE RECOVERY

After surgery, patients often feel some tightness or pressure in the chest. This feeling is typically present for the first 24-48 hours. Some degree of pain is usually associated with the surgery because of the stretching of the breast and chest tissues. Dr. Messa will prescribe pain medication to alleviate any pain or discomfort you experience. Dr. Messa may also request that you keep your arm extensions to a minimum, raising your elbows no higher than your armpit, for two or three days after surgery. Patients are examined the next day after surgery to remove the initial bandage and place the patient in a sports bra. Many times, patients are required to wear a support bra or compression bandage to help support and position the breasts. Depending on implant position, a bra may or may not need to be worn postoperatively- this will be determined by Dr. Messa.

The breasts may feel tight at first as the skin adjusts to the new breast size, and the breasts themselves may seem harder at first, though they do soften in time. Swelling is common and usually begins to fade within a few weeks. Increased sensitivity to physical contact of the breasts may occur for up to a month. The scars will be pink right after surgery and for several weeks, but will fade over time.

The amount of time it takes to recover will be different for each patient and for each technique. However, most patients find that they can return to work a few days after surgery. Nonetheless, you will need to refrain from overly strenuous activities for couple weeks.
